# Create and publish a form

> Create a contact, sign-up or survey form from a template, add your privacy policy URL, publish it, and handle the answers that arrive.

Create forms for inquiries, document requests, event sign-ups and surveys, and accept answers at a public page URL. You can work through the answers by marking them New, In progress or Done.

## Before you begin

- Add the **Forms** service. Owners and admins can add it from the **Services** screen.
- Forms and answers are saved in your organization's dedicated storage (a separate database for each organization). Adding Forms sets it up automatically, and it's ready in about a minute (until then you'll see **Setting up dedicated storage**, and the page switches over on its own). If you see **Couldn't set up dedicated storage**, an owner or admin selects **Try again**. Each person can have it set up automatically for up to 3 organizations. If you need more, or it still doesn't work after trying again, contact support.
- Members and above can create and change forms. Only owners and admins can delete a form.

## Create a form

1. On the Forms screen, select **New form**.
2. Choose a template (Contact, Document request, Event sign-up, Survey, or Blank).
3. Enter a **Form name** and select **Create**. The name is only for telling forms apart in the app. Respondents don't see it.
4. On the **Fields** tab, change the heading, description and fields. The preview on the right shows what respondents will see.
5. Select **Save changes** at the bottom.

The field types are Short text, Long text, Email, Phone, Number, URL, Dropdown, Single choice, Multiple choice, Date, Consent checkbox and Hidden value. A hidden value is filled from `?key=value` in the public page URL. Use it to give each ad or flyer its own URL and record where answers came from.

## Publish a form

1. On the **Settings** tab, enter the **Privacy policy URL**. You need it to publish. It appears as a "Privacy" link above the submit button.
2. If you need to, set the submit button text, the message on the done screen, the page to go to after sending, the time to stop accepting answers, and the maximum number of answers.
3. Select **Publish** at the top right.

Once published, the **Share** tab shows the hosted page URL and a QR code. You can send the URL, post it on social media or LINE, or put the QR code on a flyer.

The hosted page has three screens: enter, review and done. If you don't need the review screen, turn off **Show a review step** in Settings.

## Handle answers

- When an answer arrives, mukumo emails the organization's owners and admins (you can turn this off in Settings). The link in the email opens that answer.
- On the **Answers** tab, you can see the list of answers. Select a row to open its contents on the right.
- Under **Status**, switch between New, In progress and Done. The number of new answers appears next to the form in the left menu.
- **Export CSV** saves all answers in a format you can open in a spreadsheet.

## Stop accepting answers

Select **Stop accepting** at the top right, and the hosted page shows "Not accepting answers". Select **Reopen** to accept answers again. If you set a time to stop accepting answers and a maximum number of answers, the form stops automatically.

## Answer limits

Your plan sets how many answers you can accept per month and how many forms you can create. When you reach a limit, the hosted page shows "We can't accept answers right now."
